Starting at DayBreak Senior Services is simple, supportive, and designed to help our participants feel comfortable from the very beginning.
Our goal is to make sure every participant’s experience is safe, joyful, and tailored to who they are, not just their care needs.

Here’s how to begin your DayBreak journey:


1. Complete the “Get to Know Me” Profile

The first step in joining DayBreak is helping us get to know you. This short online form captures interests, background, routines, and care preferences and everything we need to begin creating meaningful, individualized days.

2. Schedule a Tour and Intake Assessment

Come see what makes DayBreak special!
We invite families and future participants to visit, meet our team, and experience the warmth and connection of our daytime community through a relaxed, personalized tour where we answer questions and help everyone feel comfortable and at ease. During this time, we will also conduct a brief intake assessment to determine eligibility and ensure we are able to meet all care needs safely and appropriately.


3. Provider Medical Assessment

Because DayBreak operates under a social model of care and is not a medical facility, all health information must be verified by a participant’s healthcare provider. Your provider will complete our Medical Provider Assessment, which ensures we can safely meet all care needs within our setting. If a recent visit includes the necessary information, the provider can complete the form based on that; otherwise, they may ask for a short appointment.
(Assessments must be updated annually or following any significant health changes to maintain enrollment.)


4. Creating Your DayBreak Experience

Once your provider’s completed assessment is received, we will connect by phone or in person to create your DayBreak experience. During this time, we will work together to confirm details, finalize your schedule, and ensure all information is accurate and up to date. We will also create an individual service plan to ensure that all care needs, preferences, and information from the Get to Know Me profile are clearly documented and communicated with our care team. This collaborative step helps us curate a meaningful, safe, and individualized experience from the very first day.


5. Begin Your DayBreak Experience

After your planning conversation, families will complete the admission packet, we will set a start date, and welcome you to DayBreak for your first day. The first day is always complimentary so everyone can feel comfortable, confident, and supported as we begin this new chapter together.
