DayBreak is Northern Utah’s only licensed Adult Day Health Program, offering a safe, engaging daytime community for older adults. We provide social connection, meaningful activities, and respite for caregivers — all in a supportive, dementia-informed setting.

Is DayBreak a medical facility?

No. DayBreak operates under a social model of care, not a medical model. We focus on engagement, well-being, and connection rather than clinical treatment. While we maintain current health information verified by your loved one’s provider, our staff are not medical practitioners.

DayBreak Senior Services is open Monday – Friday from 9 a.m. – 6 p.m. We are traditionally closed for the following holidays. Please contact us for a list of the current year’s holiday closures.

DayBreak welcomes older adults who would benefit from a safe, engaging, and social daytime environment. Our participants include:

  • Those living with dementia, including Alzheimer’s disease, Parkinson’s disease, vascular dementia, Lewy body dementia, and frontotemporal dementia.
  • Older adults aged 65 and older (exceptions can be made for individuals with early-onset Alzheimer’s disease or other qualifying needs).
  • Adults with chronic health conditions who need additional support during the day.
  • Those who spend much of their time at home and would benefit from structure, social connection, and purposeful activities.
  • Older adults recovering from a stroke who need regular physical and cognitive stimulation.
  • Families or caregivers who need daytime respite and peace of mind while their loved one is cared for in a supportive, dementia-informed setting.

Participant safety and well-being are our top priority. DayBreak may not be appropriate for individuals who:

  • Require full-time one-on-one supervision or care at all times (we maintain a 1:6 staff-to-participant ratio).
  • Exhibit aggressive or violent behavior toward themselves, others, or property.
  • Demonstrate severe exit-seeking behaviors that cannot be redirected.
  • Require assistance from more than one person to transfer from a wheelchair.
  • Are unable to eat independently (staff can assist with cutting food, adaptive utensils, and verbal cues).
  • Need incontinence care but refuse assistance.
  • Are at serious risk of self-harm or suicide.
  • Have medical or behavioral conditions that compromise their safety or the safety of others.

Each prospective participant is evaluated individually to determine eligibility and ensure DayBreak can safely meet their care needs.

How do I sign up?

Getting started at DayBreak is simple and collaborative.

  1. Complete the Get to Know Me Profile — this short form helps us learn about your loved one’s preferences, background, and what matters most to them.
  2. Schedule a Tour — come see our program in action and experience our welcoming, social environment.
  3. Have your provider complete the Medical Provider Assessment — we’ll fax the form to your provider for verification of current health and safety information.
  4. Creating Your DayBreak Experience — we’ll connect by phone or in person to finalize the schedule, confirm details, and create an individual service plan that documents all care needs and preferences.
  5. Begin your DayBreak experience — the first day is always complimentary to help your loved one feel comfortable and confident.

This process typically takes 5–7 days, depending on how quickly provider information is received.

What kind of care do you provide?

Our trained team of Certified Nursing Assistants offers assistance with personal care, mobility, and incontinence support, while focusing on dignity, comfort, and independence. We are not a medical facility and do not provide skilled nursing care, but we ensure each participant’s health and safety needs are appropriately supported within our scope.

Can you give medication?

DayBreak can store, document, and prepare medications for self-administration but cannot administer medications directly. Only medications listed and verified on the Medical Provider Assessment may be managed while attending the program. Additional approvals will be required for medications. 

What activities do you offer?

Each day at DayBreak is thoughtfully designed by our Certified Therapeutic Recreation Specialist (CTRS) to support the five domains of health — physical, cognitive, emotional, social, and spiritual well-being. Our goal is to create joyful, engaging days that nurture the whole person.

Participants enjoy a variety of therapeutic and enriching experiences, such as:

  • Cognitive activities that stimulate memory, problem-solving, and creativity through games, discussion groups, trivia, and brain challenges.
  • Physical movement through chair exercises, walking groups, dance, and outdoor activities that promote strength, balance, and confidence.
  • Emotional wellness activities such as art, music, reminiscence, and sensory experiences that help participants express themselves and find joy in familiar moments.
  • Social connection through group games, intergenerational visits, and shared meals that foster friendship and belonging.
  • Spiritual and reflective opportunities such as gratitude circles, nature-based mindfulness in our sensory garden, or quiet moments of reflection that honor personal meaning and peace.

We also offer small-group outings through our Breakaway Program, providing opportunities for community connection and real-world engagement.

All activities are adaptable to each participant’s abilities, preferences, and comfort level — ensuring everyone can take part meaningfully, feel included, and experience a sense of accomplishment and joy.

Activities are based on what each participant likes, and they can engage in group or individual activities. Our staff will be there to help and make sure everyone feels safe and included.

We offer a simplified, all-inclusive daily rate with no hidden fees. Rates include lunch, snacks, supplies, and individualized care.

Private Pay Daily Rates:

  • $99/day for 1–4 days per week
  • $89/day for 5 days per week
  • Minimum: 1 day per week

Drop-In Rate: $125/day (based on availability)

Private pay rates are pre-billed monthly to reserve your spot. There are no long-term contracts, and scheduling is flexible based on your family’s needs.

We accept credit/debit cards, cash, and checks for all private pay participants.
DayBreak also partners with:

  • The Veterans Administration (VA)
  • Medicaid Aging Waiver, New Choices Waiver, and Alternatives Program
  • Davis and Weber County Caregiver Support Programs
    If you have long-term care insurance, it may cover adult day services — we’re happy to assist with claims and documentation.

How do you schedule participants?

Schedules are created monthly. Pre-scheduling your days ensures your reserved spot. Payment for the month is due on the first day of that month. We encourage consistent attendance to maintain routine and continuity of care.

Is there a long-term commitment? What if we need to leave?

There is no long-term commitment. Services are scheduled month-to-month, and you may discontinue participation at any time with written notice. We do not issue refunds for unused days.

Our team will communicate proactively if care needs change or if we believe a higher level of support may be required. We’ll work closely with caregivers to ensure a smooth transition and connect you with trusted local resources.

Do you provide transportation?

DayBreak partners with Suzy’s Senior Companionship Care to provide door-to-door transportation within Davis and Weber Counties. We also provide group transportation for outings and community activities.

How do you communicate with families?

Communication is at the heart of quality care. We share updates through monthly newsletters, and direct conversations with caregivers. You’re always welcome to reach out with questions or updates about your loved one’s care needs.

Do you offer caregiver support or education?

Yes. DayBreak partners with local caregiver support programs to provide workshops, education, and resources for families. We also participate in community initiatives such as the Caregiver Coalition and educational series throughout Davis and Weber Counties.
